Thomas is a 56-year-old male presenting with patient to be informed of medication error causing bleeding. Admitted for DVT, prescribed heparin. Received 10x dose for 4 hours due to nursing error. Developed GI bleed, needed 2 units blood. Your task is to take a focused history from this patient.
