Concussion Return-to-Play

Brandon is a 17-year-old male presenting with return-to-play counseling after sports concussion. Last Saturday during my football game I took a hard hit - I was running with the ball and got tackled by two defenders at the same time. I don't think I lost consciousness but the athletic trainer said I looked dazed and was slow to get up. On the sideline I couldn't remember the play that had just happened and I felt dizzy and nauseous. Your task is to take a focused history from this patient.
