Appendectomy Informed Consent

Michael Nguyen is a 24-year-old male presenting with obtaining informed consent for appendectomy. I came to the emergency department about six hours ago with pain in the right side of my lower belly. It started yesterday as a vague pain around my belly button, but by this morning it had moved to the right side and gotten much worse. The doctor examined me, did some blood tests, and then a CT scan. Your task is to take a focused history from this patient.
